For carers and management teams, this workspace brings together recent information about older adults from conversation records, health indicators and items needing attention. The caregiver home shows cards for the current care circle: review basic information, recent summaries and concerns first, then open an individual's details. This sequence helps carers prepare context before a conversation and identify records that need further checking during handover.
Administrators can inspect individual, conversation, summary and concern records in more detail.
The showcase also includes a health management interface prototype using sample information. It demonstrates profiles, trend review and adding, editing or deleting medication records to support discussion of workflows and interface design. Its people, charts and prompts do not represent actual care outcomes or all functions connected to the working management application. The working application provides access to connected records according to permissions. Summaries and analyses support communication and review; they do not replace professional diagnosis or medical judgment.
I independently defined the product and information architecture, built the frontend and backend, and handled deployment and iteration. I developed a workflow that consolidates conversations and health records into structured AI analysis and stored results, with scoped access, human follow-up and report exports.
