This app brings health measurements, medication information, reminders, and a care circle together for older adults, family members, and caregivers who need to keep ongoing records. It focuses on everyday care tasks: recording a measurement, setting a reminder for an agreed schedule, and returning to recorded information when checking on someone. Family members can discuss the same records instead of relying only on memory to describe a particular day’s measurements.
Health entries support blood pressure and pulse, fasting and post-meal blood glucose, and weight, with a selectable recording time.
For medication, users first maintain the medicine’s information and then add a reminder with the dose, unit, times, frequency, and start and end dates. This turns an already agreed medication schedule into daily reminders. The app records the user’s plan; it does not select medicines or change dosages. Whether a reminder appears also depends on device notification settings and the app’s operation.
Invitations establish relationships in the care circle. Family members or caregivers can open a particular member’s page to view their health and medication records and review measurements by date. Separate member views help identify the right person when caring for several relatives. The app supports routine recordkeeping, family communication, and reference during a medical visit. Visible information depends on what has been entered and can be shared; medical interpretation of measurement changes remains a matter for qualified professionals.
I independently owned the product from care workflow definition and interaction design through the mobile application, supporting backend, deployment and ongoing development. I connected device reading recognition, user confirmation and care circle sharing into practical care workflows, alongside medication records, reminders and voice interaction.
